Description of Services: Supports for Adults living with prediabetes or diabetes

Education Clinics provide:
*certified diabetes educators , registered dieticians and nurses who work in cooperation with physicians, pharmacists and family members
  • individual and group appointments
    *referrals and monthly telehealth sessions with specialists for advanced care

Education on:
  • diabetes self-management
  • healthy living and eating
  • hyperglycemia (high blood sugar)
  • hypoglycemia (low blood sugar)
  • common emotions
  • physical activity
  • medications
