Description of Services: Convalescent care for people who no longer need to be in hospital but are unable to return to their homes in the community
  • multidisciplinary team made up of a physician, nurses, physiotherapists, occupational therapists, pharmacist, dietitian, social worker, discharge planners and volunteers
  • access to x-ray and ultrasound department, lab and emergency department, as well as medical specialists like orthopedic surgeons
Requirements
Eligibility - Population(s) Served: Persons coming out of the hospital but need assistance with their care of activities of daily living, who required therapeutic services to improve their strength, mobility and daily functions as well as those recovering from an illness or surgery and requiring additional time to convalesce
