| Description of Services: | Provides city-wide mobile intensive case management for people with frequent re-admissions to a withdrawal management system and/or Hospital Emergency Departments. Assist with system navigation services and offers assessment and referral, advocacy, support within housing, financial trusteeship, and ongoing addiction counselling and support, as well as access to mental health, psychiatry and primary care support. | ||
