Description of Services: Provides safe, professional in home health care for anyone who can't easily access * works with other community agencies to find solutions to health care needs with the goal of in home independence * Services include:
  • wellness/safety checks
  • post hospital discharge follow up
  • point of care testing
  • wound treatment
  • long term care waitlist support
  • 12 lead ECGs
  • disease education/information
  • medication check/review
  • Ontario Telemedicine Network (OTN) appointment support
  • remote medical monitoring
  • referrals for healthcare services
