Description of Services: Tele-cardiopulmonary rehab services for cardiac and pulmonary exercise class participants, in collaboration between the University of Ottawa Heart Institute, St Francis Memorial Hospital and the Lanark Renfrew Lung Program:
  • ongoing clinical cardiac rehab training
  • connects via Ontario Telehealth Network to OHI for pre-designated exercise sessions
  • education and resources for patients
  • cardiac rehabilitation nurses report, monitor, and evaluate participants’ progress
  • development of an individualized plan
  • self-management counselling
  • risk factor reduction
  • cardio-pulmonary conditioning
  • follow-up and collaboration with other care team members
