Central Intake and Referral to community lung health education and support for people living with Asthma, bronchiectasis, chronic obstructive pulmonary disease (COPD) or Interstitial Lung Disease (ILD) Services provided by Certified Respiratory Educators such as Nurse Practitioners and Registered Respiratory Therapists and include:
Services offered throughout the Counties of Lanark, Leeds and Grenville, Renfrew, Prescott-Russell, Stormont, Dundas and Glengarry, as well as in Cornwall and Ottawa Several service locations: Arnprior, Athens, Barry’s Bay, Beachburg, Bourget, Brockville, Carleton Place, Cobden, Cornwall, Crysler, Eganville, Embrun, Gananoque, Golden Lake, Kemptville, Killaloe, Lanark, Limoges, Merrickville, Ottawa, Pembroke, Petawawa, Portland, Prescott, Renfrew, Smiths Falls and Westport * Services can vary by location, contact the program for more information Accepting referrals by fax and Ocean eReferral Network |
