  • autism diagnosis

Requirements
Fees: Set fees * Covered by the Ontario Health Card
Eligibility - Population(s) Served: Children and youth 17 years and under who have:
  • missed important developmental milestones, like sitting or talking

  • complex physical and cognitive health issues

  • simple rehabilitation needs, like temporary treatment after a sports injury

and families who may need a break from caring for their child or youth with complex needs
Application: Fill out the Access Team's referral form found on the website. Send the completed form by fax or mail to the Access Team
