CardioPrevent Program - Free personalized program that can help reduce the risk for heart disease * Individuals work with a Health Coach to learn lifestyle and behavioural strategies to help them manage their risk factors * referral required CardioPrevent Postpartum Program - For women who experienced a hypertensive disorder of pregnancy or GDM, in a pregnancy within the last 5 years * referral required Women@Heart - Free peer support program led by women with heart disease, for women with heart disease |
