| Eligibility - Population(s) Served: | Seniors 70 years and older in the West End of the Ottawa Region, who have had 1 or more indoor and/or outdoor falls in the last year, who live in their own home or retirement home, who are not currently receiving services through Ontario Health atHome or a day hospital, and are willing and able to participate in taking action to reduce their risks of falls | ||
| Application: | Referrals are accepted from any source, call to obtain referral form or to complete intake over the phone * The program services the West end region of the City of Ottawa - to check catchment eligibility, call the program | ||
