Description of Services: Mental health consultation services to the elderly * services offered in private homes, nursing homes and long-term care facilities * treats people with complex geriatric issues who are also facing diminishing autonomy
  • assessment and consultation
  • support and counselling to the elderly and to family members
  • support with behaviour management (aggression, depressed mood, paranoia, etc.)
  • review of medication
  • follow-up after hospitalization
  • support and training for counsellors and caregivers who look after the elderly

Eligibility - Population(s) Served: Individuals 65 years and older living in Prescott-Russell and under the care of a family physician, as well as their families and caregivers
Application: Self-referrals accepted * friends and family members, physicians, health care professionals, and community organizations may also refer clients * for all new referrals, call central intake, toll free 1-844-304-1414
