Requirements
Fees: Set fees * Covered by the Ontario Health Card
Eligibility - Population(s) Served: Persons 18 years and older with chronic and/or persistent mental illness * also, persons who have frequent or prolonged admissions to hospitals for mental health reasons

Persons who have a significant and apparent functional disorder that is defined as: the inability to keep up with daily activities, to ensure personal safety and to participate in existing services
