Team includes Stroke Neurologists, Stroke Prevention Nurse/Nurse Specialists, and Administrative Assistants Investigations and laboratory procedures available on site * both pharmacological and non-pharmacological interventions provided to high-risk individuals and stroke survivors * specialist referrals |

Application: | Referral by an emergency room physician, primary care provider or another medical specialist * in order to ensure efficiency of the clinic visit, some tests may be completed prior to the initial visit * referral form available online | ||
